A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF SAN RAFAEL
DECLARING USI NORTHERN CALIFORNIA/DAUGHERTY & COMPANY AS
THE CITY'S INSURANCE BROKER OF RECORD
WHEREAS, it has been determined to be in the best interest of the City of
San Rafael and its employees to designate a broker of record for its group life and
long term disability insurance programs; and
WHEREAS, it has been determined that USI Northern California/Daugherty
& Company is competent to provide such services;
NOW, THEREFORE, BE IT RESOLVED that effective February 1, 2000,
the Council of the City of San Rafael does hereby designate USI Northern
California/Daugherty & Company as the City's group insurance broker of record.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby certify
that the foregoing resolution was duly and regularly introduced and adopted at a
regular meeting of the City Council of said City held on Tuesday, January 18,
2000 by the following vote, to wit:
AYES: COUNCILMEMBERS: Cohen, Heller, Miller, Phillips & Mayor Boro
NOES: COUNCILMEMBERS: None
ABSENT: COUNCILMEMBERS: None
