A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF SAN RAFAEL TO CHANGE
THE CITY'S LONG TERM DISABILITY AND TERM LIFE INSURANCE PROVIDERS TO
CANADA LIFE INSURANCE COMPANY
WHEREAS, the City Council has directed staff to continue to monitor the value of
group benefits provided to eligible employees and
WHEREAS, staff has, with the assistance of the City's designated Insurance
Broker, completed a comparison study on the premium rate increase implemented by the
City's group Long Term Disability Insurance provider; and
WHEREAS, staff has determined that the City can obtain a lower premium rate by
combining the group life insurance and the group long term disability insurance without
adversely affecting the benefit levels of the enrolled employees.
NOW, THEREFORE, BE IT RESOLVED that the City Council of the City of San
Rafael authorizes staff to proceed with changing the City's group life and long term
disability providers to Canada Life Insurance.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby certify that the
foregoing resolution was duly and regularly introduced and adopted at a regular meeting
of the City Council of said City held on Monday, April 3, 2000 by the following vote, to
wit:
