A RESOLUTION AUTHORIZING THE SIGNING OF A
CONTRACT, LEASE OR AGREEMENT
THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES as follows:
The City Manager is authorized to execute on behalf of the City of San Rafael a
contract, lease or agreement with:
California State Department of Education for $15,691 in Expansion Funds
for Child Care Scholarships (Contract GSAC-6034)
(Effective from 10/1/96 through 9/30/97)
a copy of which is hereby attached and by this reference made a part hereof.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby certify that the
foregoing resolution was duly and regularly introduced and adopted at a REGULAR

LOCAL AGREEMENT FOR CHILD DEVELOPMENT SERVICES

By signing this contract and returning it to the State, you are agreeing to provide services in accordance with the FUNDING TERMS
and CONDITIONS (FTBC)which are attached and by this reference incorporated into this contract, and the CURRENT APPLICATION
attached to the Contractor's 1995/96-96/97 GSAC contract effective May 15, 1996 through June 30, 1997 which by this reference
is incorporated into this contract. The FT&C and Requirements specify the contractual responsibilities of the State and the contractor.
This contract is effective from October 1. 1996 through September 30, 1997. For satisfactory performance of the required services,
the contractor shall be reimbursed in accordance with page 15 of the FTC, at a rate not to exceed $ 19.4 per child per day of full-time
enrollment and a Maximum Reimburable Amount (MRA) of $ 15,691 . Up to $ 2,354 of the MRA may be expended as a
one -time -only start-up costs in accordance with Education Code Section 8275 and the Funding Terms and Conditions.
Expenditure and Services provided from these funds shall be reported to the Education Finance Division (EFD) on the EFD/CD
9516-D report form for the periods ending December 31, 1996, March 31, 1997, June 30, 1997 and September 30, 1997. All reports
must be received in the Education Finance Division by the 20th of the month following the end of the reporting period. Expenditures
Drovided through June 30, 1997 shall be included in your 1996/97 audit due November 15. 1997. Expenditures and services provided
oetween July 1, 1997 and September 30, 1997 shall be included in the 1997/98 audit due November 15, 1998. Contractor shall
report on the basis of a five-part day.
Any provision of this contract found to be in violation of Federal or State statute or regulation shall be invalid but such a finding
shall not affect the remaining provisions of this contract.
