LEASE OR AGREEMENT THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES as follows: The MAYOR and CITY CLERK are authorized to execute, on behalf of the City of San Rafael a contract, lease or agreement with : State Department of Education for 1995-96 Child Development Contract with State Department of Education Amendment # 1 for Growth/Expansion Funds for $1,771 a copy of which is hereby attached and by this reference made a part hereof. I, JEANNE M. Growth LOCAL AGREEMENT FOR CHILD DEVELOPMENT SERVICES CONTRACTOR'S NAME: CITY OF SAN RAFAEL DATE: July 1, 1995 CONTRACT NUMBER GLTK-5052 PROGRAM TYPE: Extended Day Care (Latchkey) PROJECT NUMBER: 21-N916-00-03252-5 This agreement with the State of California dated July 1, 1995 designated as number GLTK5052 and Amendment #1 (Inflation) shall be further amended in the following particulars but no others: The Maximum Rate per child day of enrollment payable pursuant to the provisions of this agreement shall be $18.80 (no change). The Maximum Reimbursable Amount (MRA) payable pursuant to the provisions of this agreement shall be amended by deleting reference to $ 76.318 and inserting $ 78.089 in place thereof. SERVICE REQUIREMENTS Minimum Child Days of Enrollment (COE) Requirement shall be amended by deleting reference to 4.059.5 and inserting 4.153.7 in place thereof. Minimum Days of Operation (MDO) Requirement shall be 247 (no change). EXCEPT AS AMENDED HEREIN all terms and conditions of the original agreement shall remain unchanged and in full force and effect.
