RESOLUTION OF THE SAN RAFAEL CITY COUNCIL CHANGING THE CITY'S LONG
TERM DISABILITY PROVIDER FROM STANDARD LIFE INSURANCE COMPANY TO
UNUM LIFE INSURANCE COMPANY OF AMERICA.
WHEREAS, it has been determined to be in the best interest of the City of San Rafael and
its employees to change its group long term disability insurance provider; and
WHEREAS, it has been determined that UNUM Life Insurance Company of America is
competent to provide group long term disability insurance;
NOW THEREFORE BE IT RESOLVED that effective April 1, 1995, the Council of the City
of San Rafael does hereby designate UNUM Life Insurance Company of America as the City's
group long term disability insurance provider.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby certify that the
foregoing resolution was duly and regularly introduced and adopted at their regular meeting of the
Council of said City on Monday, the 6th day of March, 1995, by the following vote, to wit:
