A RESOLUTION AUTHORIZING THE SIGNING
OF A CONTRACT, LEASE OR AGREEMENT
THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES as follows:
The MAYOR and CITY CLERK are authorized to execute, on behalf
of the City of San Rafael, a contract, lease or agreement with
INSURANCE CONSULTING ASSOCIATES, INC. NINETY (90) DAY CONTRACT
EXTENSION AGREEMENT or until April 30, 1992, a copy of which is
hereby attached and by this reference made a part hereof.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby
certify that the foregoing resolution was duly and regularly
introduced and adopted at a Regular meeting of the City Council of
said City held on Monday the 3rd day of February, 1992, by the
following vote, to wit:
AYES: COUNCILMEMBERS: Breiner, Cohen, Shippey, Thayer & Mayor Boro
NOES: COUNCILMEMBERS: None

CONTRACT EXTENSION AGREEMENT
IT IS HEREBY AGREED between the City of San Rafael
(hereinafter referred to as the City) and Insurance Consulting
Associates, Inc. (hereinafter referred to as the Claims
Administrator) that the "SELF-INSURED CLAIMS MANAGEMENT AND
GENERAL RISK MANAGEMENT CONSULTING CONTRACT", which originally
provided for services from October 31, 1990 through October 31,
1991, is extended to provide for continuation of services for a
period of ninety (90) days from January 31, 1992 through
April 30, 1992. There are no changes in the Contract revisions.
Contract referred to was identified in Resolution No. 8261.
