HomeMy WebLinkAboutCC Resolution 8038 (Insurance Consulting Associates)RESOLUTION NO. 8038 A RESOLUTION AUTHORIZING THE SIGNING OF A CONTRACT, LEASE OR AGREEMENT THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES as follows: The MAYOR and CITY CLERK are authorized to execute, on behalf of the City of San Rafael, a contract, lease or agreement with INSURANCE CONSULTING ASSOCIATES, INC. - CONTRACT EXTENSION AGREEMENT FOR ONE At YEAR FROM SEPTEMBER 10, 1989 AND TERMINATING AT MIDNIGHT, SEPTEMBER 9, 1990, a copy of which is hereby attached and by this reference made a part hereof. I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby certify that the foregoing resolution was duly and regularly introduced and adopted at a Regular meeting of the City Council of said City held on Monday the Eighteenth day of September, 1989, by the following vote, to wit: AYES: COUNCILMEMBERS: Boro, Breiner, Frugoli, Thayer & Mayor Mulryan NOES: COUNCILMEMBERS: None ABSENT: COUNCILMEMBERS: None JE M. LEONCI9I, City Clerk �RlG9N �;1 $o�e Original CONTRACT EXTENSION AGREEMENT IT IS HEREBY AGREED between the City of San Rafael (hereinafter referred to as the City) and Insurance Consulting Associates, Inc. (hereinafter referred to as the Claims Administrator) that the "SELF INSURED CLAIMS MANAGEMENT AND LOSS CONTROL CONSULTING CONTRACT," which originally provided for services to be rendered from 9-10-87 through 9-9-88, but which was modified and extended to provide for continuation of services for an additional year from 9-10-88 through 9-9-89, is again modified and extended for an additional year to provide for continued services from 9-10-89 through 9-9-90. There are no changes in provisions, except as follows: III. PAYMENT FOR SERVICES Professional Services will be billed at the rate of $50.00 per hour. The annual Computer Services Fee is $3,600.00. That fee covers the cost of computer hardware, space on the system, and production of the Register and Summary sections of the Loss Run. The Computer Services Fee is due in full at the inception of the contract. However, for the convenience of the City, payments of $300.00 per month may be made toward any remaining balance. IV. TERM The term of this agreement shall be one year beginning September 10, 1989, and terminating at midnight, September 9, 1990. This Contract may be terminated by either party for any reason upon thirty (30) days' written notice. The contract may be renewed subject to renegotiation of terms and conditions. XIV.
