A RESOLUTION AUTHORIZING THE SIGNING OF A
CONTRACT, LEASE OR AGREEMENT
THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES as follows:
The MAYOR and CITY CLERK are authorized to execute, on behalf of
the City of San Rafael, a contract, lease or agreement with
TEL PLUS FOR RENTAL OF SATURN II E TELEPHONE SYSTEM (TEMPnRARY
EXTEASION ON A MONTH-TO-MONTH BASIS FOR A PERIOD NOT TO EXCEED
THREE MONTHS BEGINNING FEBRUARY 1, 1989 AT A RENTAL RATE OF
$3,500 PER MONTH)
a copy of which is hereby attached and by this reference made a part
hereof.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby
certify that the foregoing resolution was duly and regularly intro-
duced and adopted at a Regular meeting of the City Council of said
City held on Monday the Sixth _ day of February
19 89 , by the following vote, to wit:

EXTENSION AND RATE AMENDMLLif
TO SYSTEM INSTALLATION AND RENTAL AGREEMENT
This Agreement made between Tel Plus, A Siemens Company, with its
principal office at 4464 A Willow Road, Pleasanton, CA 94566 and the
City of San Rafael amends the original rental agreement executed on
February 26, 1988 by Tel Plus and December 21, 1987 by the City as
follows:
Section 6. Term of Agreement Amendment:
The term of this Agreement shall be extended on a month-to-month
basis for a period not to exceed three (3) months beginning on
February 1, 1989.
Section 7. Rental Amendment:
Seller hereby rents the system equipment described in Exhibit A
hereto to Customer on the following terms and conditions:
The rental rate through the month of January 1989 shall be
$1500 per month. For each month thereafter of the
extension period a rental rate of $3500 per month or an
appropriate daily pro -rated amount for any rental period of
less than a full month shall apply.
All other terms and conditions of the original System Installation
and Rental Agreement remain in place.
