A RESOLUTION AUTHORIZING THE SIGNING OF A
CONTRACT, LEASE OR AGREEMENT
THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES as follows:
The MAYOR and CITY CLERK are authorized to execute, on behalf of
the City of San Rafael, a contract, lease or agreement with
P&F SAFETY SERVICES FOR LIABILITY SAFETY INSPECTION
INCLUDING ADDENniiM
a copy of which is hereby attached and by this reference made a part
hereof.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby
certify that the foregoing resolution was duly and regularly intro-
duced and adopted at a Regular meeting of the City Council of said

Dear Randy,
As per our most recent telephone conversation, please consider the
following as a proposal/contract.
P&F Safety Services will undertake a program of services to provide
the CITY OF SAN RAFAEL with a Safety Audit/Survey of city facilities,
to determine the extent of its potential Public Liability exposure(s).
The Auait/Survey will include, but will not necessarily be limited
to:
- City structures and grounds
- Parks grounds and equipment
- Areas of density, i.e. baseball parks,
convention/community centers, etc.
- Golf course
- Swimming pool(s)
- Airport
- Solid waste site
- Any other areas of liability exposure
It is felt that this program will involve approximately two to three
days of Audit/Survey time. Our fees are charged at $500.00 per day
plus hotel/motel charges if required. The fee includes travel,
research, dictation and transcription of findings and suggestions for
same wherever possible, of corrective actions.

ADDENDUM TO CONTRACT BETWEEN P&F SAFETY
SERVICES AND THE CITY OF SAN RAFAEL
P&F Safety Services agrees to hold harmless, indemnify
and defend the City of San Rafael, its officers and
employees from any and all claims, loss, liability
and damages arising from P&F, its agents employees'
and officers' acts or omissions arising during the
hours P&F is performing this service.
P&F Safety Services and the City of San Rafael agree
to submit any claims arising under this contract to
binding arbitration pursuant to the current provisions
of the California Code of Civil Procedure and any
successor statutes.
