A RESOLUTION AUTHORIZING THE SIGNING OF A
CONTRACT, LEASE OR AGREEMENT
THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES as follows:
The MAYOR and CITY CLERK are authorized to execute, on behalf of
the City of San Rafael, a contract, lease or agreement with
Agreement accepting San Francisco Foundation grant award for Library
Services Planning Study
a copy of which is hereby attached and by this reference made a part
hereof.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby
certify that the foregoing resolution was duly and regularly intro-
duced and adopted at a regular meeting of the City Council of said
City held on Tuesday
the third day of January ,

Grant Purpose To support a countywide comprehensive long-range plan and study for
improved library services.

Terms of Grant
I. Special Con (Iitions: The fuIIow ing special condition , if any, must be met by the grantee in order to receive the grant.
The libraries will be required to pay costs related to keeping the
services_goinq during the time staff and consultants meet for regular
review and study related activities. The libraries will also provide
50% of the costs to release staff to participate in the study.

I. Payments Payments are made on a quarterly basis

5. Audit K IN -ports The Foundation require, prnuthc
Iinancial and pr,crannnatic reports from the grantee on
the use of the• toad; n•rened I'nIt— utnerwlse indicated,
a progress report is due before the third grant payment
can be processed. and a final report is due L20 day's
after the final payment. If necessary, an audit may be
required. Payments on the grant are contingent on
the submission of progress reports and special conditions
as noted above. Guidelines on how to prepare the
reports will be mailed to your agency one month before
the reports are due
6. Monitoring & Evaluation In order to asses!; the
eiiective•ness of nor grants, the Foundation may monitor
or conduct an evaluation of the program funded by this
grunt, which may include visits by representatives of the
Foundation to observe the grunter .s program procedures
and operations and to discuss the program with the
grantee's personnel.
i. Multi -fear Grants It is the usual practice to require
an evaluation of the grantee's performance during the
first year of a grant before making payment for any
successive year.
8. Publicity We suggest that the grantee accept respon-
sibility for media coverage of the grant. Any statement
about the Foundation's policies, interests or personnel
should be cleared with the Foundation's officvs.We ask that
materials produced credit the FoundationS :,uppoa.
The Foundation's Communicutions Office would be
pleased to advise in the preparation cif a press release,
or other fortis of public information activities.
9. Reversion All funds not expended for the purposes
agreed to by the grantee and the Foundation must be
returned to the Foundation.
