A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF
SAN RAFAEL AUTHORIZING THE RENEWAL OF A CONTRACT
WITH R. L. KAUTZ AND COMPANY FOR WORKERS'
COMPENSATION CLAIMS ADMINISTRATION
WHEREAS, the City of San Rafael has contracted with
R. L. Kautz and Company since 1976 to administer its Workers'
Compensation claims; and
WHEREAS, the basic agreement was amended in January, 1983,
to permit termination of their agreement upon 60 days' notice; and
WHEREAS, R.L. Kautz and Company is requesting that their
remuneration be increased to One Hundred Thirty -Two Dollars
($132.00) per reported claim for 1985.
THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES AS FOLLOWS:
The agreement with R. L. Kautz and Company, including the
amendment approved January 17, 1983, is renewed through November 30,
1985. Remuneration is to be One Hundred Thirty -Two Dollars
($132.00) per reported claim;
BE IT FURTHER RESOLVED that the Mayor and City Clerk are
authorized to sign amendment Number II, which is attached hereto
and made a part hereof.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael,
hereby certify that the foregoing resolution was duly and regularly
introduced and adopted at a regular meeting of the Council of said
City on Monday , the 3rd day of December 1984, by the
following vote, to -wit:

AMENDMENT NUMBER II
WORKERS' COMPENSATION CLAIMS ADMINISTRATION
It is hereby agreed that the Memorandum of Agreement dated
December 1, 1976, between the City of San Rafael and R. L. Kautz
and Company is amended as follows:
(a) Effective December 1, 1984, remuneration to R. L. Kautz
and Company will be on the basis of One Hundred Thirty -Two
Dollars ($132.00) per reported claim.
(b) This agreement may be terminated by the Self -Insurer (i.e.,
the City of San Rafael) by giving to R. L. Kautz and Company
written notice of their intention to cancel this Agreement
at least sixty (60) calendar days prior to the date of
termination, and the Self -Insurer (i.e., the City of
San Rafael) shall not be liable for more than a pro -rata
share of the base fee.
(c) In the event this Aqreement is terminated by either R. L.
Kautz and Company or the Self -Insurer (i.e., the City of
San Rafael) it is expressly understood that any and all
files, documents and records pertaininq to current and
former employees are the property of the Self -Insurer
(i.e., the City of San Rafael) and are to be turned over
to them upon request.
All other items of the initial Agreement remain in full force
and effect.
This Agreement is accepted and formalized this 3rd day of
December , 1984.
