WIT N E SSE T H: For and in consideration of the mutual promises and cove- nants hereinafter contained, the parties hereto do hereby agree as follows: Section 1: Contractor shall provide advance life support and basic life support emergency ambulance service to the residents of Area as described in Exhibit A attached hereto and by this reference incorporated herein. Section 2: In consideration of the provision of such services without charge to the residents of the Area, Area will pay Contractor as follows: a) For fiscal year 1982-83, Area will pay Contractor the sum of $12, for each single-family dwelling unit within the boundaries of Area. It is understood that actual cost to Contractor for provision of paramedic service to Area for fiscal year 1982-83 is $16.40 per single-family dwelling unit. It is further understood that said sums include the (.50) per parcel service charge of the County of Marin. b) For fiscal year 1983-84, Area agrees to pay Contractor an amount per single-family dwelling unit which represents the actual cost of providing service for fiscal year 1983-84 plus a $4.40 per dwelling unit deficit from 1982-83, said sum not to exceed $21 per dwelling unit within the boundaries of Area. If said sum exceeds $21, the amount in excess of $21 shall be added to the subsequent years payment until the $4.40 deficit has been completely repaid. c) For fiscal year 1984-85 and all subsequent fiscal years, Area agrees to pay Contractor a sum per single-family dwelling unit within the boundary of Area which equals the actual cost of providing paramedic service plus any remaining balance on the $4.40 deficit wit~ the exception that in said years the total payment to Contractor per dwelling unit within Area shall not exceed $21. ~ r"",:} PI---! t,\,. ~ 1 , I. I -1 - Section 3: As used in all sections of this Agreement, the clause "actual cost of providing service" shall be as jointly agreed by Contractor and Area after at least one meeting to be held no later than June 1 of each year and to be finalized by August 1 and shall be calculated in accordance with the criteria listed in Exhibit B attached hereto and by this reference incorporated herein. Section 4: This Agreement shall continue in full force and effect through June 30, 1987. No later than May 1, 1986, Contractor agrees to meet with Area to negotiate services provided and sums payable to Contractor for fiscal year 1987-88 and beyond. Aug. 1983 EXHIBIT A • Exhibit B Format for Allocation of Paramedic Services A. Staffing Pattern. The total staffing required to provide paramedic service shall be calculated as follows: 1. Agreed upon number of posts times 24 hours times 365 days equals total hours of coverage required. 2. Total number of hours available per year per employee equals 2,912, less average annual usage for vacation, sick leave and training equals net hours available per year per employee. 3. Divide total hours of coverage by net hours available per year per employee equals the number of staff required to provide this service. B. Salaries/Benefits. The cost of salaries and benefits shall be calculated as follows: 1. The estimated annual salary for a year plus holiday pay, overtime, retirement, health/dental coverage, Worker's Compensation, uniform allowance, and edu- cational incentive equals the total annual cost per employee. 2. Total annual cost per employee times number of staff required to provide the service from Section A above equals total salary and benefit cost. C. Auxiliary Staff. Overhead costs applicable to auxiliary staff may be charged provided that the City documents the percentage of each auxiliary staff's total time spent on the paramedic program. Such auxiliary staff costs shall be calculated as follows: Percentage of time spent by each staff member times the annual salary plus benefits of that staff member. D. Services and Supplies. A line item expenditure listing for all services and supplies for the coming fiscal year shall be supplied by City along with the actual and estimated expenditures by line item for the current fiscal year. E. Total Budget for Paramedic Program 1. The total cost for the Paramedic Program shall be calculated as the sum of the total salary and benefit costs from Section B above plus total auxiliary costs from Section C above plus total services and supplies costs from Section D above. F. The cost per dwelling unit shall be calculated as follows: Total budget for Paramedic Program times the percentage of Marinwood CSD dwelling units are to the total number of dwelling units for the City of San Rafael, CSA #19, Marinwood CSD, and CSA #13. The number of dwelling unit figures shall be as supplied by the County Auditor-Controller.
