A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF SAN RAFAEL AUTHORIZING THE SIGNING OF A
CONTRACT, LEASE OR AGREEMENT
(WITH P & F SAFETY SERVICES FOR SAFETY PROGRAMS)
THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES
as follows:
WHEREAS, the Mayor and City Clerk are authorized to
execute, on behalf of the City of San Rafael, a contract, lease
or agreement with P & F Safety Services for safety programs.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael,
hereby certify that the foregoing resolution was duly and regularly
introduced and adopted at a special meeting of the Council of said
City on MONDAY , the 20th day of DECEMBER 1982, by the
following vote, to wit:

CONTRACT FOR SAFETY SERVICES
P&F SAFETY SERVICES will provide to the CITY OF SAN RAFAEL the followings
(1) The SERVICE AGENT will conduct periodic, indepth physical
surveys of the City's facilities on a regular basis.
(2) Will attend Safety Committee Meetings and conduct Training
Sessions as requested.
(3) The SERVICE AGENT in connection with these matters will
make recommendations which are consistant with applicable
Federal and State OSHA regulations.
(4) The SERVICE AGENT will act as a Safety Consultant, reference
and information source.
(5) A total of ten (10) Engineering Days will be provided during
the term of this contract. Allocated time to include travel
and reports.
(6) Be it understood that THE SERVICE AGENT (P&F SAFETY SERVICES)
is not an employee of THE CITY OF SA14 RAFAEL, but an Independent
Contractor.
Contract Term, will commence as of January 1, 1983 and will be continuous
each annual year thereafter, unless either party wishes to terminate the
Contract, by giving written notice of cancellation sixty (60) days in
advance of cancellation date,'
Service Fees for services rendered, will be in the amount of Four Thousand
Nine Hundred Fifty Dollars, ($4,950.00) annually. Fee is payable on a semi-
annual basis. Two Thousand Four Hundred Seventy -Five Dollars ($2,475.00)
at the time of the signing of this contract.

Two Thousand Four Hundred Seventy -Five Dollars ($2,475.00) due on or before
July 1, 1983. Pricing of contract each annual year is subject to increase,
based on annual cost of living index, but only upon agreement by THE CITY
OF SAN RAFAEL in advance.
