HomeMy WebLinkAboutResolution No. 5742RESOLUTION NO. 5742 A RESOLUTION AUTHORIZING THE SIGNING OF A CONTRACT, LEASE OR AGREEMENT THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES as follows: The MAYOR and CITY CLERK are authorized to execute, on behalf of the City of San Rafael a contract, lease or agreement with The San Francisco Foundation - Grant Agreement for Bahia Vista Community School, Amount of $6,000 a copy of which is hereby attached and by this reference made a part hereof. I, JEANNE M. LEONCINI, City Clerk of the City of San Rafael, hereby certify that the foregoing resolution was duly and regularly introduced and adopted ata —regular meeting of the City Council of said City held on Monday the 20th day of August 19 79 by the following vote, to wit: AYES: COUNCILMEMBERS: Breiner, Jensen, Miskimen, Nixon & Mayor Mulryan NOES: COUNCILMEMBERS: None ABSENT: COUNCILMEMBERS: None J NNE M. LEONCINI, 'ty Clerk 0-la-U!t7INAL ul THE SAN FRANCISCO FOUNDATION GRANT AGREEMENT The conditions set forth below are deemed to be agreed to by the grantee if the grantee accepts any payment against a grant. Description of Grant Grantee (Name & Address) BAHIA VISTA COMMUNITY SCHOOL Payee: 12S Bahia Way, San Rafael, CA 94901 Same as grantee Amount of Grant: $6,000 Grant Purpose: To replace funds anticipated through County contracts. To provide services and programs in a low-income neighborhood of San Rafael to deal with problems df juvenile delinquency, transiency, crime, child care, education, etc. Grant Objectives: (As listed in proposal) Reporting Schedule: Payment Schedule: Two payments; the first in September, 1979 Terms of Grant 1. USE OF Funds mist be used by the grantee strictly in accordance with the final budget on which the grant was based. Any variation from the budget must be approved in advance by the Foundation. 2. PAY'•iENTS: Payments are usually made on a quarterly basis, in advance, in the months of March, June, September, and December. The Foundation attempts to ac- commodate its payments to the convenience of the grantee. Funds are not disbursed until the grantee indicates that its expenses under the grant are about to begin -- for example, personnel to be paid under the grant have been employed. Should the program not he ready to start by the time the first payment is scheduled, payment will be delayed until the fol- lowing quarter. 3. AUDIT The Foundation requires periodic .financial and narrative reports from �U:I) the grantee covering use of funds received from the Foundation, as in- dicated in the schedule above. t:'e request that the grantee furnish to the Foundation a detailed written report on the use of the grant, in- cluding an appraisal of the program results under the grant for the re- porting period. A six-month report is due before the third payment check is released. A final report is clue 120 days after the final payment check is received. The final payment on your grant is scheduled De cembe r 1979 and the final report is due in our office by October•1980 The format of the financial section of this report s o�IT —econsistent with that of the budget as it was approved by the Distribution Committee, and show a comparison of actual to budgeted expenditures. If necessary, an audit may be required. 4. PROGWI In order to assess the offectiveness of our grants, the � INITOOIUNG Foundation may conduct an evaluation of the program funded =��ALLUMON: by this grant, which may include visits by representatives of the Foundation to observe the Grantee's program procedures and operations and discuss the program with the Grantee's personnel. S. REAT-16ION: All funds not expended for the purposes agreed to by the Grantee and the Foundation must be returned to the Foi-mda- tion. 6. PUBLIUM We suggest that the Grantee accept responsibility for press coverage of the grant. j\ny statement about the Foundation's policies, interests or personnel should be cleared with the Foundation office.
