TASK ORDER NO. 12.1 (Amendment to Task Order No. 12) Civil Discipline Smith Ranch Road/Lucas Valley Road Resurfacing THIS AMENDMENT TO TASK ORDER No. 12 is entered into on 7 , 20 between the City of San Rafael, a municipal corporation ("City"), and %eQ ("Consultant"). RECITALS A. The City and Consultant have entered into a Master Consultant Services Agreement ("Agreement") dated January 22, 2020 under which Consultant has agreed to perform professional Civil services for the City. B. Pursuant to the Agreement, on June 12, 2020, the City and Consultant entered into "Task Order No. 12" under which Consultant agreed to perform Civil services in connection with City's Smith Ranch Road/Lucas Valley Road Resurfacing Project (the "Project"), as specified in Exhibits I and 11 included in Task Order No. 12. C. City now requires additional construction management support for the Project, and Consultant is willing to provide such services. NOW, THEREFORE, City and Consultant agree to amend Task Order No. 12 as follows: AGREEMENT TO AMEND TASK ORDER NO. 12 1. Section 1 of Task Order No. 12, entitled "Scope of Services," is hereby amended to include the additional services set forth in Consultant's proposal entitled "Exhibit 2 (Revised TO #12 for Additional CM Assistance)" dated July 13, 2020, attached to this Amendment to Task Order No. 12 as "Attachment I" and incorporated herein by reference. 2. Section.' of the Task Order, entitled "Date of completion; Work hours estimate; Cost estimate and budget; Fee schedule" is hereby amended to include the additional services, work hours estimate, cost estimate and budget set forth in Consultant's proposal entitled "Exhibit 2 (Revised TO #12 for Additional CM Assistance)" dated July 13, 2020, attached to this Amendment to Task Order No. 12 as "Attachment I" and incorporated herein by reference. 3. F,xcept as specifically amended herein, all of the other provisions, terms and obligations of Task Order No. 12 dated June 12, 2020 shall remain valid and shall be in full force.
