TASK ORDER NO. 55.1 (Amendment to Task Order No. 55) Inspection Services Discipline Bungalow and Woodland Resurfacing (City Project No. 11397) Contract Amount: $69,247.92 THIS AMENDMENT TO TASK ORDER No. 55 is entered into on Ox� , 20,17--2— between the City of San Rafael, a municipal corporation ("City"), and Ghirardelli Associates, Inc., ("Consultant"). RECITALS A. The City and Consultant have entered into a Master Consultant Services Agreement ("Agreement") dated June 7, 2021 under which Consultant has agreed to perform professional inspection services for the City. B. Pursuant to the Agreement, on September 20, 2021, the City and Consultant entered into "Task Order No. 55" under which Consultant agreed to perform inspection services in connection with City's Bungalow and Woodland Resurfacing project (the "Project"), as specified in Exhibit A attached to and included in Task Order No. 55, for a total not to exceed 62,329.92. C. City now requires inspection services for the Project, and the Consultant is willing to provide such services. NOW, THEREFORE, City and Consultant agree to amend Task Order No. 55 as follows: AGREEMENT TO AMEND TASK ORDER NO. 55 1. Section 1 of Task Order No. 55, entitled "Scope of Services," is hereby amended to include the additional services set forth in Consultant's proposal dated February 17, 2022, attached to this Amendment to Task Order No. 55 as Exhibit A and incorporated herein by reference. 2. Section 2 of Task Order No. 55, entitled "Date of completion; Work hours estimate; Cost estimate and budget; Fee schedule" is hereby amended to include the additional services, work hours estimate, cost estimate and budget set forth in Consultant's proposal dated February 17, 2022, attached to this Amendment to Task Order No. 55 as Exhibit A, for an amount of $6,918, changing the total not to exceed to $69,247.92. 3. Except as specifically amended herein, all of the other provisions, terms and obligations of Task Order No. 55 dated September 20, 2021, shall remain valid and shall be in full force. Rev. Ghirardelli estimates that our fee will be $6,918.00. The fee is based on full time inspection hours for 5 days for our Construction Inspector, Jose Garcia.
