TASK ORDER NO. 80 Inspection Services Discipline Project title: 2021/22 Slurry Seal Inspection and Materials Testing Contract Amount: $74,896 THIS TASK ORDER is entered into on U 6�-202_ between the City of San Rafael, a municipal corporation ("City"), and Ghirardeffi Associates, Inc., ("Consultant"). RECITALS A. The City and Consultant have entered into a Master Consultant Services Agreement ("Agreement") dated June 7, 2021 under which Consultant has agreed to perform certain professional consulting services for the City; and B. Pursuant to the terns of the Agreement, the City wishes Consultant to perform certain tasks as set forth in this Task Order, which constitute the Project; NOW, THEREFORE, City and Consultant agree as follows: AGREEMENT 1. Scope of services. Consultant shall provide any and all of the services set forth in Exhibit A of this Task Order, dated June 3, 2022, as directed by the City. Charles Krueger shall be the Project Manager for Consultant for this Task Order. Additional professional services may be performed by other Consultant staff with prior notice and approval by City. a. City's duties. The City will: (1) Determine the scope of services, as set forth in Exhibit A. (2) Make available the project plans and specifications and other City data related to the project, including full infornation as to the City's requirements. (3) Designate Shawn Graf as the person at the City responsible for monitoring performance under this Task Order. b. Consultant duties. The Consultant will: (1) Provide and perfonn tasks identified in Exhibit A, Scope of Services. (2) Provide miscellaneous consulting and other services related to the Project and not stipulated elsewhere in this Task Order, when requested by the City in writing. A Task Order Amendment is required if these additional services cannot be performed within the Task Order budget. Rev. May 2021 City of San Rafael Master Consultant Services Agreement Task Order 2. Date of completion; Work hours estimate-, Cost estimate and budget; Fee schedule. With regard to each project given to Consultant that is subject to this Task Order, the time of completion, work hours estimate, cost estimate and budget and fee schedule are set forth in Exhibit A to this Task Order. 3. Authorization to proceed. The City will give Consultant authorization to proceed with the work specified in this Task Order only after Consultant submits updated insurance certificates and endorsements, in accordance with Section 10 of the Agreement. 4. Master Agreement teens; Conflicts. Except as expressly otherwise provided herein, all terms of the Agreement shall apply to Consultant in the performance of work pursuant to this Task Order. If there is a conflict between this Task Order and the Agreement, this Task Order shall control. 4. Amendment of Task Order; Conflicts. Except as provided in Section 1(b)(2) above, this Task Order may not be modified except by an Amended Task Order executed and approved in the same manner as the Task Order. 5. Si agr] tures. (Ghirardelli) to provide inspection and materials testing services to the City of San Rafael (City) for slurry seal and pavement preservation projects in 2022 and 2023. Ghirardelli's pavement preservation specialist, Cesar Lara, will utilize his many years of experience to provide inspection on the City's pavement preservation projects. Mr. Lara will also assist with submittal reviews, attend meetings as needed, review as -built drawings and perform other tasks as requested by the City. Ghirardelli will also provide additional inspectors if needed, under the guidance of Mr. Lara. Ghirardelli will utilize the resources of our materials testing subconsultant, G3 Quality, Inc. to perform quality assurance testing for the City's pavement preservation projects. G3 Quality is currently providing similar services for the County of Marin. SCOPE OF WORK Ghirardelli will provide construction inspection and quality assurance materials testing services on the project, including, but not limited to the following: • Inspect and observe construction contractor's work daily for compliance with the contract documents, environmental documents, state laws, regulations, etc. • Provide construction contractor with acceptability or deficiencies in work and required actions for contract compliance. • Maintain daily inspection reports of work, including photos, descriptions of daily activities, any corrective actions to be taken by the Contractor, persons and equipment onsite, and any other pertinent information. • Review of materials and equipment delivered to site for compliance with submittals and contract documents. • Coordinate and provide quality assurance material testing as required. • Review of construction contractor's compliance with all permits, stormwater regulations and permits (SWPPP), and environmental documents. These activities will be performed on an as -needed basis as directed by the City. Mr. Lara's inspection services will be provided at the hourly rates shown below. Hourly rates for additional inspectors, if needed, will be provided to the City for approval. The Not -To -Exceed budget for inspection and materials testing services is $74,896.00.
