HomeMy WebLinkAboutDS Technical Training & Consulting 2015; Amendment 10551 Biel 13 100 W AGREEMENT FOR PROFESSIONAL SERVICES FOR TECHNICAL TRAINING AND CONSULTING This Amendment is made and entered into this ;U4t- day of December, 2015, by and between the CITY OF SAN RAFAEL (hereinafter "CITY"), and Quest Media and Supplies, Inc., (hereinafter "CONTRACTOR"). RECITALS WHEREAS, the CITY and the CONTRACTOR entered into an original Agreement for Professional Services for Technical Training and Consulting on September 15, 2015 (the "Agreement"); and WHEREAS, paragraph 5 of the original Agreement gives the City Manager of the CITY the authority to approve an extension to the term of the Agreement for a period of up to 12 months; and WHEREAS, the CITY wishes to extend the term of the Agreement through June 30, 2016; and WHEREAS, CONTRACTOR also agrees to extend the term of the Agreement through June 30, 2016; AMENDMENT NOW, THEREFORE, the parties hereby agree to extend the term of the Agreement through June 30, 2016, with no other changes to the terms and conditions of the Agreement. EPSTEIN, Cijy Att4n PROFESSIONAL SERVICES AGREEMENT/CONTRACT COMPLETION CHECKLIST AND ROUTING SLIP Below is the process for getting your professional services agreements/contracts finalized and executed. Please attach this "Completion Checklist and Routing Slip" to the front of your contract as you circulate it for review and signatures. Please use this form for all professional services agreements/contracts (not just those requiring City Council approval). This process should occur in the order presented below. Step Responsible Description Completion Department Date 1 City Attorney Review, revise, and comment on draft agreement. 2 Contracting Department Forward final agreement to contractor for their signature. Obtain at least two signed r' originals from contractor. 3 Contracting Department Agendize contractor -signed agreement for N/A Council approval, if Council approval necessary (as defined by City Attorney/City 4 City Attorney Ordinance*). Review and approve form of agreement; bonds, and insurance certificates and lzq),s- endorsements. 5 City Manager / Mayor / or Agreement executed by Council authorized Department Head official. 6 City Clerk City Clerk attests signatures, retains original , agreement and forwards copies to the contracting department.
