RESOLUTION OF THE CITY COUNCIL OF THE CITY OF SAN
RAFAEL APPROVING AND AUTHORIZING THE CITY MANAGER
TO EXECUTE TWO AGREEMENTS BETWEEN THE CITY OF
SAN RAFAEL AND CIGNA INSURANCE: 1) RENEWAL OF THE
AGREEMENT TO PROVIDE ALL ELIGIBLE EMPLOYEES BASIC LIFE
INSURANCE, DEPENDENT LIFE INSURANCE, VOLUNTARY LIFE
INSURANCE AND LONG-TERM DISABILITY INSURANCE (January 1,
2008 through December 31, 2010), AND 2) AN EXTENSION AGREEMENT
TO PROVIDE BASIC ACCIDENTAL DEATH AND DISMEMBERMENT
INSURANCE AND VOLUNTARY ACCIDENTAL DEATH AND DIS-
MEMBERMENT INSURANCE (January 1, 2008 through June 30, 2010)
WHEREAS, the City received numerous responses to its RFP to companies providing
employee insurance benefits, including CIGNA which currently provides those benefits to City
employees; and
WHEREAS, CIGNA responded with the best rates and offered to continue its current
agreements with the City for employee insurance benefits for an additional three (3) year term
without any additional increase in rates:
WHEREAS, the City's employee benefits committee reviewed all the proposals and
found the CIGNA proposal to provide the best insurance coverage at the lowest cost and,
accordingly, recommended that the City renew and extend its employee insurance benefit
coverage with CIGNA;
NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF SAN RAFAEL DOES
HEREBY RESOLVE AS FOLLOWS:
1. The City Council approves and authorizes the City Manager to execute, on behalf of
the City of San Rafael, the renewal of an agreement between Cigna Insurance
Company and the City of San Rafael to provide Basic Life Insurance, Dependent Life
Insurance, Voluntary Life Insurance and Long -Term Disability Insurance for eligible
City employees effective January 1, 2008 through December 31, 2010, in a form to be
approved by the City Attorney; and further,
2. The City Council approves and authorizes the City Manager to execute, on behalf of
the City of San Rafael, an extension of the agreement with Cigna Insurance Company
to provide Basic Accidental Death and Dismemberment Insurance and Voluntary
Accidental Death and Dismemberment Insurance effective January 1, 2008 through
June 30, 2010, in a form to be approved by the City Attorney.

foregoing Resolution was duly and regularly introduced and adopted at a regular meeting of the
City Council of said City held on Mondav, the third day of December, 2007 by the following
