RESOLUTION OF THE CITY COUNCIL OF THE CITY OF SAN RAFAEL
AUTHORIZING THE USE OF FACSIMILE SIGNATURES FOR THE
WESTAMERICA BANK WORKERS' COMPENSATION TRUST ACCOUNT
WHEREAS, the City of San Rafael is a municipal corporation organized and existing
under the laws of the State of California; and
WHEREAS, the City desires to authorize the use of facsimile signatures for the
workers' compensation trust account.
NOW, THEREFORE, BE IT RESOLVED that WESTAMERICA BANK is hereby
authorized and directed to honor as genuine and authorized instruments of this municipal
corporation any and all checks, drafts or other orders for the payment of money drawn in the
name of the municipal corporation and signed with the authorized facsimile signatures.
BE IT FURTHER RESOLVED that the City of San Rafael assumes entire
responsibility of the use of actual or purported facsimile signatures of any person or persons
named in the Authorization to Use Facsimile signature form appearing on checks, drafts or
orders of this municipal corporation drawn on WESTAMERICA BANK, and for payments made
by said bank in reliance thereon, which payment may be charged to this municipal corporation,
regardless of whom or by what means the actual or purported facsimile signature(s) are affixed,
if they resemble the facsimile specimen(s), duly certified to or filed with WESTAMERICA
BANK by the City Clerk or other officer of this municipal corporation.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby certify that the
foregoing resolution was duly and regularly introduced and adopted at a regular meeting of the
Council of said City the 3rd day of May, 2004, by the following vote, to wit:

NOES: COUNCILMEMBERS: None

1/We declare that Westamerica Bank be, and it hereby is, authorized and directed to
honor as genuine and authorized instruments any and all checks, drafts or other orders
for payment of money drawn in the name of City of San Rafael on account number
5-0/ ZZ and signed with the facsimile signature(s) of any of the
following:
