Agreements Between The City of San Rafael and Cigna Insurance:
1) Renewal of the Agreement to Provide all Eligible Employees Basic
Life Insurance, Dependent Life Insurance, Voluntary Life Insurance
and Long-Term Disability Insurance (January 1, 2008 Through
December 31, 2010); and
2) An Extension Agreement to Provide Basic Accidental Death and
Dismemberment Insurance and Voluntary Accidental Death and
