RESOLUTION AUTHORIZING INSTALLATION OF
SUPPLEMENTARY DRAINAGE FACILITIES ON "C"
STREET IN CONJUNCTION WITH THE MEYER
ROAD OFF SITE SUBDIVISION IMPROVEMENTS
WHEREAS, the bid of Ghilotti Brothers Construction, Inc., dated
April 7, 1994, at the unit prices stated in its bid submitted to Armand L. Locke,
was and is lowest and best bid for said work and said bidder is the lowest
responsible bidder; and
WHEREAS, the unit prices for the supplementary drainage facilities
requested by the Department of Public Works were reviewed by the Land
Development Division and were found to be fair and reasonable.
NOW, THEREFORE, IT IS HEREBY ORDERED AND RESOLVED by the
San Rafael City Council that the bid of Ghilotti Brothers Construction, Inc.
submitted to Armand L. Locke, pertaining to supplementary drainage facilities
in conjunction with the Meyer Road off site subdivision improvements is hereby
accepted at said price of $18,421.00, and that the contract for said work and
improvements be and same is hereby accepted at the price mentioned in said
bid.
IT IS FURTHER ORDERED AND RESOLVED that the cost for said
supplementary drainage facilities be increased to $20,000.00 to cover the cost
of survey staking and other contingency costs, and that the construction cost
for said facilities be appropriated from the Miscellaneous Right of Way Gas Tax
Fund.
I, JEANNE. M. LEONCINI, Clerk of the City of San Rafael, hereby
certify that the foregoing resolution was duly and regularly introduced and
adopted at a regular meeting of the Council of said City on Monday, the 2nd day
of May, 1994, by the following vote, to wit:

1. Item CA "Digout & Repair" will be paid for actual quantity of work required.

3. No costs have been included for temporary erosion control. We feel that the project will be completed prior
to the onset of the rainy season.
4. We have included costs to air and mandel test and TV the sanitary lines. We have not included costs to do
the same to the storm drain lines.
5. Prices* for waterline are based on Sheet 6 of b titled "Schematic Drainage and Utility Plan".
6. We believe there is the potential for substantial cost savings if the flowline of the 24" RCP can be raised.

This proposal subject to our approval of financial arrangements or fund set aside.
In the event that it becomes necessary for Contractor, by lien or other action, to enforce collection of any amount
payable by Buyer hereunder, Buyer agrees to pay Contractor all expenses, including interest and attorney's fees,
incurred in the institution and prosecution of such action.
Contractors are required by law to be licensed and regulated by the Contractors' State License Board. Any questions
concerning the responsibilities of a contractor may be referred to the Registrar of the Board whose address is:
Contractors' State License Board, 1020 N. Street, Sacramento, California 95814.

All material is guaranteed to be as specified. All
work to be completed in a workmanlike manner
according to standard practices. Any alteration or
deviation from above spec-ifications involving extra
costs will be executed only upon written orders, and
will become an extra charge over and above the
estimate. All agreements contingent upon strikes,
accidents or delays beyond our control. Owner to
carry fire, tornado and other necessary insurance.
Our workers are fully covered by Workmen's
Compensation Insurance.
