HomeMy WebLinkAboutResolution No. 5869RESOLUTION NO. 5869 RESOLUTION OF THE CITY COUNCIL OF THE CITY OF SAN RAFAEL AUTHORIZING CITY OF SAN RAFAEL TO APPLY FOR AND ACCEPT SAN FRANCISCO FOUNDATION FUNDING FOR 1980-1984 WHEREAS, The San Francisco Foundation has available funds for public and social services in Marin County; and WHEREAS, The San Rafael Public Library aims to serve the total community of San Rafael; and the Library Outreach Program has been working for several years to satisfy this goal; and WHEREAS, the community in the form of individual library patrons, service clubs, social service agencies, officials in the field of aging and retirement facility staffs are desirous and supportive of the con- tinuation of this program; and WHEREAS, The San Rafael Public Library is desirous of making application to the San Francsico Foundation to extend and improve services to the people of San Rafael. NOW, THEREFORE, BE IT RESOLVED that the City of San Rafael be authorized to apply for and accept local funding for 1980-1984 as provided by grant award(s) approved by the San Francisco Founda- tion, and the Mayor of San Rafael is hereby authorized to sign sub- sequent agreements for approved grant award(s). LEONCINI, City Clerk GRAFT AGREET624T The conditions set forth below are deemed to be agreed to by the grantee if the grantee accepts any payment against a grant. Description of Grant Grantee (Name & Address) San Rafael Public Library 1100 E Street, San Rafael, CA. 94901 Payee: City of San Rafael, 1400 Fifth Ave., San Rafael, CA. 94102 Amount of Grant: $148,719 / 24 months Grant Purpose: To support a county -wide library outreach program to serve institutionalized and homebound elderly & disabled library patron; Grant Objectives: To increase individual access to books and special materials. Reporting Schedule: Progress report prior to 3rd quarterly, final at end of each grant year. Pavnent Schedule:_ Q,lartPrl v beginning September, 1980. Terms of Grant 1. USE OF Funds must be used by the grantee strictly in accordance with the final budget on which the grant was based. Any variation from the budget must be approved in advance by the Foundation. 2. PA)NU7_N'TS: Payments are usually made on a quarterly basis, in advance, in the months of T9arch, June, September, and December. The Foundation attempts to ac- commodate its payments to the convenience of the grantee. Funds are not disbursed until the grantee indicates that its expenses under the grant are about to begin -- for example, personnel to be paid under the grant have been employed. Should the program not be ready to start by the time the first payment is scheduled, payment will be delayed until the fol- lowing quarter. 3. AIIPIT VM Id TORTS : The Foundation requires periodic financial and narrative reports from the grantee covering use of funds received from the Foundation, as in- dicated in the schedule above. We request that the grantee furnish to the Foundation a detailed written report on the use of the grant, in- cluding an appraisal of the program results under the grant for the re- porting period. A six-month report is due before the third payment check is released. A final report is due 120 days after the final payment check is received. The final payment on your grant is scheduled June, 1982 and the final report is due in our office by g The format of the financial section of this report should a consistent with that of the budget as it was approved by the Distribution Committee, and show a comparison of actual to budgeted expenditures. If necessary, an -audit may be required. 4. PROGRATi In order to assess the effectiveness of our grants, the FUTITM NG Foundation may conduct an evaluation of the program funded F, UATION: by this grant, which may include visits by representatives of the Foundation to observe the Grantee's program procedures and operations and discuss the program with the Grantee's personnel. 5. RE1'ERSION: All funds not expended for the purposes agreed to by the Grantee and the Foundation must be returned to the Founda- tion. 6. GRANTS FOR It is the usual practice to require an evaluation of the f - 71f Grantee's performance during the first year of a grant be - ONE YEAR: fore making payment for any successive year. 7. PUBLICITY: We suggest that the Grantee accept responsibility for press coverage of the grant. Any statement about the Foundation's policies, interests or personnel should be cleared with the Foundation office. We would be pleased to advise in the preparation of a press release. 8.
