MASTER CONSULTANT SERVICES AGREEMENT
DATED JANUARY 22, 2020
WITH BELLECCI & ASSOCIATES, INC.
TASK ORDER NO. 12

THIS TASK ORDER is entered into on 1/(A.h e 49 , 2&—'-&etween the City of San
Rafael, a municipal corporation ("City"), and Bellecci & Associates, Inc. ("Consultant").
RECITALS
A. The City and Consultant have entered into a Master Consultant Services Agreement
("Agreement") dated January 22, 2020 under which Consultant has agreed to perform certain professional
consulting services for the City; and
B. Pursuant to the terms of the Agreement, the City wishes Consultant to perform certain
tasks as set forth in this Task Order, which constitute the Project;
NOW, THEREFORE, City and Consultant agree as follows:
AGREEMENT
1. Scope of services. Consultant shall provide any and all of the services set forth in Exhibit I of
this Task Order, dated May 22, 2020, as directed by the City.
Robert Broestl shall be the Project Manager for Consultant for this Task Order. Additional professional
services may be performed by other Consultant staff with prior notice and approval by City.
a. City's duties. The City will:
(1) Determine the scope of services, as set forth in Exhibit I.
(2) Make available the project plans and specifications and other City data related to the
project, including full information as to the City's requirements.
(3) Designate Hunter Young as the person at the City responsible for monitoring
performance under this Task Order.
b.Consultant duties. The Consultant will:
(1) Provide and perform tasks identified in Exhibit I, Scope of Services.
(2) Provide miscellaneous consulting and other services related to the Project and not
stipulated elsewhere in this Task Order, when requested by the City in writing. A Task
Order Amendment is required if these additional services cannot be performed within the
Task Order budget.

Task Order
The work hours estimate, cost estimate and budget and fee schedule are set forth in the attachments to this
Task Order, as follows:
Scope of Services: Exhibit I
Date of completion: Exhibit II
Work hours estimate: Exhibit II
Cost estimate and budget: Exhibit II
3. Authorization to proceed.
The City will give Consultant authorization to proceed with the work specified in this Task Order only after
Consultant submits updated insurance certificates and endorsements, in accordance with Section 10 of the
Agreement.
4. Master Agreement terms; Conflicts.
Except as expressly otherwise provided herein, all terms of the Agreement shall apply to Consultant in the
performance of work pursuant to this Task Order. If there is a conflict between this Task Order and the
Agreement, this Task Order shall control.
Amendment of Task Order: Conflicts.
Except as provided in Section 1(b)(2) above, this Task Order may not be modified except by an Amended
Task Order executed and approved in the same manner as the Task Order.
Signatures. The parties have executed this Task Order, as of the date first written above.

Project Understanding
We understand that the City of San Rafael would like Bellecci & Associates to provide
construction inspection for the construction of the Smith Ranch Road / Lucas Valley
Resurfacing Project (11336). It is understood that the City will be providing the
construction management and administration for the Project.
We will provide a qualified inspector to perform a wide range of services including but
not limited to: field inspections, daily inspection reports, quality assurance, materials
verifications, traffic control review and document control. Our inspector will be
responsible for providing services during construction and post -construction for the
project proposed projects
Project Approach
We anticipate that the services our inspector will provide for the Construction
Inspection Services for the Smith Ranch Road / Lucas Valley Road Resurfacing Project
(11336) will include, but not be limited to the following:
Task 1 Construction Inspection Services
We will be responsible for providing construction inspection services during the
construction throughout the duration of the project.
a. Perform field inspections and other quality control/assurance activities;
b. Maintain a daily inspector's report that records the hours worked by labor
detail and equipment used. Detail must be sufficient to permit the review of the
contractor's costs of the work in a manner similar to force amount. Equipment
must be identified sufficiently to enable determination of the applicable rental
rates and operator(s) minimum wage. The narrative portion of the report shall
include a description of the contractor's operation and location of work and
any other pertinent information;
c. Maintain and organize photographic and video records;
d. Remark the pavement repair markings as based on the plans and any visible
marks on the pavement.
e. Review and inspect contractor's work for compliance with approved contract
documents and specifications on a daily basis;
f. Monitor corrective actions taken by the contractor for fixing work that is not in
compliance with the contract documents;
g. Provide acceptable public relations by answering questions from the public;
h. Prepare and submit field inspection diaries daily;
i. Capture digital photographs of daily work in progress;
j. Attend meetings with the City as requested;
k. Review contractor's compliance with workplace safety and health standards
and notification of City of non-compliance;

1. Oversee contractor's traffic control, detours, land closures, and staging plans;
in. Perform site coordination between contractor and City staff and affected utility
companies.
n. Coordinate and observe all field testing such as filed materials testing services
(by others)
Task 2 Post -Construction Services
We will be responsible for providing substantial and final completion services.
a. Perform site inspection(s) to determine if the facility(s) are complete and in
compliance with the contract documents;
b. Prepare Punch -List and inspect Punch -List corrective actions in consideration
of final project acceptance. Also, oversee "as -built" information between the
contractor and design engineer;
c. Provide recommendations to City to release of payments and retention to
contractor.
Office Support and Management
Our assigned construction administration assistant will type up, proof-read and add
selective photos to the inspector's hand written daily report. The finalized daily
inspection report with photos will be provided to the City in an electronic PDF format.
Our project manager will provide support only as requested and on an as -needed basis
for the project inspector by consulting on field related issues that arise during the
construction activity.
