Members of the public may speak on agenda items.
An opportunity for public comments will be provided prior to the Board beginning its discussion on each
item.
Any records relating to an Agenda Item, received by a majority or more of the board or commission less than 72 hours before the meeting, shall
be available for inspection in the CDD Dept, at 1400 Fifth Ave, Third Floor, San Rafael, CA

2. 800 Mission Ave. (Formerly 1203-1211 Lincoln Ave.) – Review of final details on the
proposed site landscaping, exterior lighting and outdoor terrace finishes of an
approved 88-bed/77-suite assisted living facility with memory care services (“Aegis
San Rafael”) located on two vacant parcels at the northwest corner of Lincoln and
Mission Ave. A Use Permit and an Environmental and Design Review Permit were
approved for the project, subject to conditions, on September 4, 2018; APNS: 011 -184-
08 & -09; Multifamily Residential – High Density (HR1) District Zone; Geoff Forner,
Applicant; ASG San Rafael, LLC, Owner; Downtown Neighborhood. Project Planner:

3. 400-496 Las Gallinas Ave. (Northgate III Shopping Center) – Environmental and Design
Review, Sign Program Amendment and Exception for Northgate III improvements. The
project proposes an update to the façade, amendment to the existing Sign Program
and a reduction in the backup dimension for several parking spaces for a new
sidewalk on the site; APN: 175-250-14; General Commercial (GC) Zoning District; Jon
