MASTER CONSULTANT SERVICES AGREEMENT

Contract Amount: $73,438
THIS TASK ORDER is entered into on l , 202 /between the City of San
Rafael, a municipal corporation ("City"), and Park Engiiieering, Inc. ("Consultant").
RECITALS
A. The City and Consultant have entered into a Master Consultant Services Agreement
("Agreement") dated December 17, 2019 under which Consultant has agreed to perform certain
professional consulting services for the City; and
B. Pursuant to the terms of the Agreement, the City wishes Consultant to perform certain
tasks as set forth in this Task Order, which constitute the Project;
NOW, THEREFORE, City and Consultant agree as follows:
AGREEMENT
1. Scope of services. Consultant shall provide any and all of the services set forth in Exhibit A of
this Task Order, dated October 7, 2021, as directed by the City.
Steve Patterson shall be the Project Manager for Consultant for this Task Order. Additional professional
services may be performed by other Consultant staff with prior notice and approval by City.
a. City's duties. The City will:
(1) Determine the scope of services, as set forth in Exhibit A.
(2) Make available the project plans and specifications and other City data related to the
project, including full information as to the City's requirements.
(3) Designate Alysia Reyes as the person at the City responsible for monitoring performance
under this Task Order.
b.Consultant duties. The Consultant will:
(1) Provide and perform tasks identified in Exhibit A, Scope of Services.
(2) Provide miscellaneous consulting and other services related to the Project and not
stipulated elsewhere in this Task Order, when requested by the City in writing. A Task
Order Amendment is required if these additional services cannot be performed within the
Task Order budget.

2. Date of completion: Work hours estimate; Cost estimate and budget; Fee schedule.
With regard to each project given to Consultant that is subject to this Task Order, the time of completion,
work hours estimate, cost estimate and budget and fee schedule are set forth in Exhibit A to this Task Order.
3. Authorization to proceed.
The City will give Consultant authorization to proceed with the work specified in this Task Order only after
Consultant submits updated insurance certificates and endorsements, in accordance with Section 10 of the
Agreement.
4. Master Agreement terms; Conflicts.
Except as expressly otherwise provided herein, all terms of the Agreement shall apply to Consultant in the
performance of work pursuant to this Task Order. If there is a conflict between this Task Order and the
Agreement, this Task Order shall control.
4. Amendment of Task Order; Conflicts.
Except as provided in Section 1(b)(2) above, this Task Order may not be modified except by an Amended
Task Order executed and approved in the same manner as the Task Order.
Signatures. The parties have executed this Task Order, as of the date first written above.

Dear Alysia,
Park Engineering, Inc. is pleased to submit this proposal to provide construction inspection
services for the City's B Street Box Culvert Repair Project. We appreciate the opportunity to
continue to work with the City on this project.
Anthony Goryl, PE, will be the Construction Inspector for the City. Anthony has spent over 7
years in the construction industry working for many Bay Area city and county public works
departments and transportation agencies. He has a significant background specific to
structural construction and rehabilitation projects. Anthony has excellent public relation
skills and will work closely with local business and homeowners.

SCOPE OF SERVICES
Park Engineering will support the City by providing construction inspection for this Box
Culvert Replacement Project. We will perform all the duties required, including, but not
limited to the following:
• Perform all field inspection activities to monitor compliance with the contract plans and
specifications. Record all items of work, labor, equipment, materials incorporated,
materials tested, traffic control utilized and any other pertinent information in a daily
diary for permanent record.
• Monitor all field work for contract compliance and document with daily reports and
pictures
• Coordinating with the contractor for all aspects of the work including traffic control,
utility work, safety, and ensuring contract for acceptance of completed work by the City
• Work closely with City staff on project schedule, field progress and issues
• Prepare supporting quantity calculations for work performed, including any contract
change orders, and progress pay estimates on a monthly basis

Key Qualifications
Anthony is a professional engineer with over seven years of diverse experience emphasizing in
transportation structures, and performing roles from project engineer to project manager. Experienced
in planning, analysis, design (developing plans, specs, and estimates), load rating, preventive
maintenance, inspection and rehabilitation, and construction oversight and support. Additional project
experience includes seismic retrofit, analysis and design of waterfront structures, and inspection of
hydraulic/flood control facilities. Accomplished projects under both design -bid -build and design -build
delivery methods. Experienced in the application of advanced engineering software for nonlinear and
seismic analysis. Extensive knowledge of design, construction, inspection, and load rating standards
such as Caltrans Engineering Manuals, AASHTO LRFD and MBE, and FHWA. Solid understanding and
application of project estimating and scheduling tools such as Caltrans Cost Database and Microsoft
Project. Strong in written and oral communication, and outstanding in data research (technical and
non-technical). Willingness to continue learning and developing both personally and professionally.
Ability to self -teach and become knowledgeable with previously unfamiliar projects and concepts.

Involved with over 60 projects located within various states and countries all within seven years of
hire. Responsible for marketing and developing business with existing and new clients. Managed and
involved with multiple simultaneous projects of varying size and complexity. Trained and mentored
younger staff members.
Bridge related work includes analysis, design, load rating, preventive maintenance, inspection and
rehabilitation, peer review, and design support during construction. Developed plans, specifications,
and estimates for local and regional agencies from short simple -span slabs under design -bid -build
procurement to cast-in-place/prestressed box girders over 400 -feet in length under the design -build
delivery method. Load rated bridges constructed of various materials such as stress -laminated
timber to multi -span reinforced splayed box girders. Experienced in preventive maintenance
treatments and developing preventive maintenance plans for local agencies to receive HBP funding.
Provided emergency repair services for private and local agencies, including repair work on an aging
timber railroad trestle. Inspected local and state agency bridges following National Bridge Inspection
Standards and Element Level Inspection such as balanced and unbalanced steel swing -span bridges.
Provided reviews for construction activities such as shop drawings and general submittals.
Non -bridge work includes analysis and design of waterfront structures, design of soundwalls, design
of retaining walls, seismic retrofit of the Transbay Tube, and inspection of hydraulic flood control
facilities. Used advanced analysis software to design waterfront structures under static, transient,
inertial, and kinematic loads (up to six feet of soil displacement). Designed soundwalls for local
Cities. Designed pile supported and spread footing cantilever retaining walls and tieback walls.
Performed inspection and condition assessments for local flood control agency facilities including
drop structures, energy dissipators, and concrete channels

Superstructure Field Engineer — Managed and planned the erection of 17 precast girders for the
Angle Lake Station as part of the South 200th Link Extension in SeaTac, WA. Performed quality
control inspection services of the precast girders at the casting plant. Supported superstructure and
substructure crews by performing field inspection and quality control checks. Provided quality
control for post -tensioned duct grouting, span -by -span and balanced cantilever erection, mass
concrete pier table pours, and column pours. Performed post erection quality control checks such as
mapping balanced cantilever cracking post -stressing. Developed and submitted RFIs. Documented
and organized construction materials such as prestressing strands, rebar, and embeds for
integration into 3D BIM modeled. Aided in concrete finishing such as repair of spalls and
honeycombs.
Casting Yard Field Engineer
Aided in casting yard setup including erecting rebar jigs, casting machines, rubber tire gantry cranes,
and segment storage. Construction work plans were developed to meet productivity goals and
outline.
Estimating Intern
Quantity take -offs of construction materials used for bidding the California High Speed Rail project,
including MSE & CIP retaining walls, trench sections of the rail alignment, trackway earthwork,
roadway removal, drainage materials and other bid items related to the section. Reconciled bid
items for consistent estimating between members of the Joint Venture for the California High Speed
Rail. Helped to improve relationships and communication between PCL and DBE businesses
