MASTER CONSULTANT SERVICES AGREEMENT
DATED JANUARY 22, 2020
WITH BELLECCI & ASSOCIATES
TASK ORDER NO. 72

THIS TASK ORDER is entered into onA.Jr� 310 , 20 between the City of San
Rafael, a municipal corporation ("City"), and Bellecci & Associates, Inc., a corporation authorized to do
business in California ("Consultant").
RECITALS
A. The City and Consultant have entered into a Master Consultant Services Agreement
("Agreement") dated January 22, 2020 under which Consultant has agreed to perform certain professional
consulting services for the City; and
B. Pursuant to the terms of the Agreement, the City wishes Consultant to perform certain
tasks as set forth in this Task Order, which constitute the Project;
NOW, THEREFORE, City and Consultant agree as follows:
AGREEMENT
1. Scope of services. Consultant shall provide any and all of the services set forth in Exhibit A of
this Task Order, dated February 25, 2022, as directed by the City.
Robert Broestl shall be the Project Manager for Consultant for this Task Order. Additional professional
services may be performed by other Consultant staff with prior notice and approval by City.
a. City's duties. The City will:
(1) Determine the scope of services, as set forth in Exhibit A.
(2) Make available the project plans and specifications and other City data related to the
project, including full information as to the City's requirements.
(3) Designate Alysia Reyes as the person at the City responsible for monitoring performance
under this Task Order.
b. Consultant duties. The Consultant will:
(1) Provide and perform tasks identified in Exhibit A, Scope of Services.
(2) Provide miscellaneous consulting and other services related to the Project and not
stipulated elsewhere in this Task Order, when requested by the City in writing. A Task
Order Amendment is required if these additional services cannot be performed within the

Task Order budget.
Date of completion: Work hours estimate, Cast estimate and budget: Fee schedule.
With regard to each project given to Consultant that is subject to this Task Order, the time of completion,
work hours estimate, cost estimate and budget and fee schedule are set forth in Exhibit A to this Task Order.
Authorization to proceed.
The City will give Consultant authorization to proceed with the work specified in this Task Order only after
Consultant submits updated insurance certificates and endorsements, in accordance with Section 10 of the
Agreement.
Master Agreement terms: Conflicts.
Except as expressly otherwise provided herein, all terms of the Agreement shall apply to Consultant in the
performance of work pursuant to this Task Order. If there is a conflict between this Task Order and the
Agreement, this Task Order shall control.
Amendment of Task Order; Conflicts.
Except as provided in Section 1(b)(2) above, this Task Order may not be modified except by an Amended
Task Order executed and approved in the same manner as the Task Order.
5. Signatures. The parties have executed this Task Order, as of the date first written above.

Regarding: Proposal to provide Engineering Design Services for the 120 Santa Margarita
Bubble Up Drainage System Repair Project.
Dear Ms. Reyes:
Bellecci & Associates appreciates the opportunity to work with the City of San Rafael. Per
your email received on February 2, 2022, and subsequent follow-up emails, Bellecci &
Associates Inc. proposes to provide engineering design services to the City of San Rafael for
the above project.
Attached for your review is our proposed scope of services (Exhibit 1). Also attached is a
copy of our proposed budget to do the work (Exhibit 2).
Bellecci & Associates, Inc. proposes to provide the services outlined in this proposal for an
estimated fee of $33,356.00 to be invoiced monthly on an hourly basis (reasonable
reimbursable expenses are included in the hourly rates). We will not exceed this fee without
prior approval from the City.

Project Understanding
EXHIBIT I
The City of San Rafael seeks surveying and engineering services for the 120 Santa Margarita
Bubble up Drainage System Repair Project. We understand that during rainstorms that happened
in late 2021 the existing bubble up storm drain system got clogged with leaves and debris
causing the pipe near the bubble up outfall structure to expand and damage the concrete
driveway located at 120 Santa Margarita. The project consists of the investigating the existing
condition of the storm drain pipe and inlet/outlet structures, designing a repair for the current
storm drain system and show repairs for the damaged driveway. The exhibit below shows the
location of the proposed work.
We will prepare design review submittals at the 90% and 100% stages. The scope of work for the
above projects is described in more detail below:

Scope of Work:
EXHIBIT I
PROJECT COORDINATION MEETINGS AND PROGRESS REPORTS
Bellecci will attend meetings with City staff throughout the design process. The meetings shall
include:
Two (2) design review meetings with Public Works staff.
In addition, Bellecci will provide project progress updates on a monthly basis. The report will
include accomplishments, anticipated problems or issues, schedule updates and schedule of
upcoming events. In addition, we will provide cost reports with every invoice that highlights the
approved budget, budget remaining, and amount spent for the invoicing period, estimated cost at
completion and an estimate of the percent complete.
Deliverable: Progress reports
STORMDRAIN CCTV VIDEO INVESTIGATION
Bellecci will contract with a CCTV inspection company to clean & video inspect the storm drain
line in this project. We will review the CCTV information for the storm drain system to identify
the existing condition of the storm drain pipe and the extent of the damaged pipe.
Deliverable: Electronic Copy of the Storm Drain CCTV Investigation
DETAILED SITE INVESTIGATION
Bellecci will visually inspect the project site to assess and accomplish the following:
■ Determine the existing constraints that may impact the construction of the storm drain lines
• Evaluate the condition of the existing inlet & outlet structures
■ Evaluate the condition and extend of the surface damage
■ Determine the limits of work and identify possible conform issues
• Identify visible utilities (valves, manholes, monuments, etc.)
Deliverable: None
TOPOGRAPHIC SURVEYS
Using GPS Datum information, Bellecci's survey crews will collect necessary field topographic
information, utility locations and street grades to supplement and complete the base information for
the design of the project. The survey work will be performed and tied into the Cal coordinates
Zone III NAD83/NAVD88 State Plane system.
Deliverable: Electronic Copy (PDF) of the Topographic Survey
COORDINATION WITH UTILITIES
Bellecci will coordinate with owners of the utilities (including City owned facilities) in the
project areas to obtain their utility base maps for use in developing the plans. Utility locations
will be shown on the base map used for the replacement of the sewer pipe and electrical conduit.

DETAILED PLANS, SPECIFICATIONSAND ESTIMATES
EXHIBIT 1
The plans, specifications and bid items will be compiled in standard City format providing sufficient
information to obtain comprehensive contractor bids and to construct the various projects. The
plans will be prepared in ACAD and use the City base map and aerial photographs as the design
base maps supplemented with relevant ground shot survey information. The plans will be at a
P= 10' scale and will include a profile of the existing storm drain line to be replaced. Bellecci will
provide the City with 90% submittals and the final bid set. Engineer's estimates will be provided
with each submittal.
Deliverable: 2 sets of Plans, Specifications and Estimates at the 90% and Final Bid Set on bond
and electronic files.
BID SUPPORT
During the bidding phase, Bellecci will attend the prebid meetings and will provide the City with
assistance to answer any Contractor questions pertaining to the plans and specifications. We will
prepare and issue contract addenda, as needed. We will assist the City in the review of bids,
reference checks, and preparation of award documents.
Deliverable: Copies of all addenda and correspondence
CONSTRUCTIONSUPPORT SERVICES
Bellecci will attend the preconstruction meeting with the contractor, City staff and applicable utility
companies. We will review the contractor submittals for compliance with the specifications and
respond within five business days. We will also clarify questions regarding the plans (RFIs), review
change order requests from the contractor and prepare change orders.
