TASK ORDER NO. 90.1 (Amendment to Task Order No. 90) 76 Albert Park Lane (WildCare) Planning Application Review Amendment Amount: $4,882.50 THIS AMENDMENT TO TASK ORDER No. 90 is entered into on 05/23/2024 between the City of San Rafael, a municipal corporation ("City"), and Pakpour Consulting Group, Inc., ("Contractor"). RECITALS A. Whereas Master Consultant Services Agreement is hereby renamed Service Level Agreement; and B. The City and Contractor (as may be identified as "Consultant" in the original Agreement) have entered into a Service Level Agreement ("Agreement") dated August 20, 2021 under which Contractor has agreed to perform certain contract services for the City; and C. Pursuant to the Agreement, on February 23, 2023, the City and Contractor entered into "Task Order No. 90" under which Contractor agreed to perform contract services in connection with City's 76 Albert Park Lane (Wildcare) Planning Application Review (the "Project"), for a total not to exceed $8,820; and D. City now requires additional services for the Project, and the Contractor is willing to provide such services. NOW, THEREFORE, City and Contractor agree to amend Task Order No. 90 as follows: AGREEMENT TO AMEND TASK ORDER 1. Section 1 entitled "Scope of Services," is hereby amended to include the additional services set forth in Exhibit A of this Task Order Amendment. 2. Section 2 entitled "Date of completion; Work hours estimate; Cost estimate and budget; Fee schedule" is hereby amended to include the additional services, work hours estimate, cost estimate and budget set forth in Exhibit A, for an amount not to exceed $4,882.50, changing the total not to exceed to $13,702.50. 3. Except as specifically amended herein, all of the other provisions, terms and obligations of Task Order No. 90 shall remain valid and shall be in full force. Rev. This proposal was prepared to capture plan review services for the WildCare project beyond the hours stated in the original proposal. The previous Task Order (Task Order #90) was budgeted for 40 hours for the first and second submittal reviews. Due to the complexity of the project due to the flood elevation, off -site parking, and the proximity to San Rafael Creek and the pedestrian bridge, the project required additional review time above a typical plan review. Scope of Work The additional hours that were not billed with the original Task Order #90 include project management and file setup, initial coordination with City of San Rafael, proposal preparation, internal quality review, phone calls with the City, meetings and meeting preparation related to the off -site parking, meeting with City staff via conference call, pedestrian bridge coordination, review of the hydraulic analysis memo and finalizing Cycle 2 review comments. Budget Senior Engineer 19.5 hours at $220 per hour = $4,290.00 Project Engineer 2 hours at $180.00 = $ 360.00 5% Direct Cost Multiplier — $ 232.50 Total = $4,882.50 We are proposing a budget in an amount not to exceed $4,882.50 and billed on a time and materials basis.
