AGREEMENT FOR PROFESSIONAL SERVICES
FOR TECHNICAL TRAINING AND CONSULTING
This Amendment is made and entered into this ;U4t- day of December, 2015, by and
between the CITY OF SAN RAFAEL (hereinafter "CITY"), and Quest Media and Supplies, Inc.,
(hereinafter "CONTRACTOR").
RECITALS
WHEREAS, the CITY and the CONTRACTOR entered into an original Agreement for
Professional Services for Technical Training and Consulting on September 15, 2015 (the
"Agreement"); and
WHEREAS, paragraph 5 of the original Agreement gives the City Manager of the CITY
the authority to approve an extension to the term of the Agreement for a period of up to 12 months;
and
WHEREAS, the CITY wishes to extend the term of the Agreement through June 30, 2016;
and
WHEREAS, CONTRACTOR also agrees to extend the term of the Agreement through
June 30, 2016;
AMENDMENT
NOW, THEREFORE, the parties hereby agree to extend the term of the Agreement
through June 30, 2016, with no other changes to the terms and conditions of the Agreement.
IN WITNESS WHEREOF, the parties have executed this Amendment as of the day,
month and year first above written.

executed. Please attach this "Completion Checklist and Routing Slip" to the front of your
contract as you circulate it for review and signatures. Please use this form for all professional
services agreements/contracts (not just those requiring City Council approval).
