A RESOLUTION AUTHORIZING THE SIGNING OF A
CONTRACT, LEASE OR AGREEMENT
THE CITY COUNCIL OF THE CITY OF SAN RAFAEL RESOLVES as follows:
The MAYOR and CITY CLERK are authorized to execute, on behalf of
the City of San Rafael, a contract, lease or agreement with
INSURANCE CONSULTING ASSOCIATES, INC. CONTRACT MODIFICATION
AGREEMENT (3 months, or until September 9, 1990)
a copy of which is hereby attached and by this reference made a part
hereof.
I, JEANNE M. LEONCINI, Clerk of the City of San Rafael, hereby
certify that the foregoing resolution was duly and regularly intro-
duced and adopted at a Reaular meeting of the City Council of said
City held on Mondav the 2nd day of Julv
19 90 , by the following vote, to wit:

CONTRACT MODIFICATION AGREEMEN-.
IT IS HEREBY AGREED between the City of San Rafael (hereinafter referred to as
the City) and Insurance Consulting Associates, Inc. (hereinafter referred to as
the Claims Administrator) that the "SELF INSURED CLAIMS MANAGEMENT AND LOSS
CONTROL CONSULTING CONTRACT" between the City and the Claims Administrator
which is scheduled to terminate at midnight, September 9, 1990, is hereby
modified with no changes in terms, except as follows:
III. PAYMENT FOR SERVICES
The annual Computer services fee is $4,800.00. That fee covers the cost of
computer hardware, space on the system, and production of the Register and
Summary sections of the Loss Run. The Computer Services Fee is due in full
at the inception of the contract, though for the convenience of the City,
payments of $400.00 per month may be made toward any remaining balance.
III A. The following additional services shall be included:
1. Updating of subsequent loss run data by review of incoming
correspondence;
2. Completion of computer input forms;
3. Periodic review of in-house files to insure that the dates of
rejection were entered and statute of limitations were met;
4. Periodic review of in-house files to insure that payments and
closings have been entered;
5. Periodic review and update of loss run data contained in the
hard copy files being handled by the City of San Rafael.
IV. TERM
The term of this agreement shall be one year beginning June 14, 1990, and
terminating at midnight, September 9, 1990. This Contract may be terminated
by either party for any reason upon thirty (30) days written notice. The
Contract may be renewed subject to renegotiation of terms and conditions.
XIV. SPECIAL PROVISIONS
This paragraph is deleted in its entirety under the renewal contract.
