TASK ORDER NO. 11 Culvert Inspection and Repair Strategy for Lateral under B Street THIS TASK ORDER is entered into on —JLkN`Q. g , 20�obetween the City of San rk Thomas & Rafael, a municipal corporation ("City"), and MaCompany, Inc., ("Consultant"). RECITALS A. The City and Consultant have entered into a Master Consultant Services Agreement ("Agreement") dated December 18, 2019 under which Consultant has agreed to perform certain professional consulting services for the City; and B. Pursuant to the terms of the Agreement, the City wishes Consultant to perform certain tasks as set forth in this Task Order, which constitute the Project; NOW, THEREFORE, City and Consultant agree as follows: AGREEMENT 1. Scope of services. Consultant shall provide any and all of the services set forth in Attachment A of this Task Order, dated May 19, 2020, as directed by the City. Julie Passalacqua shall be the Project Manager for Consultant for this Task Order. Additional professional services may be performed by other Consultant staff with prior notice and approval by City. a. City's duties. The City will: (1) Determine the scope of services, as set forth in Attachment A. (2) Make available the project plans and specifications and other City data related to the project, including full information as to the City's requirements. (3) Designate Theo Sanchez as the person at the City responsible for monitoring performance under this Task Order. b.Consultant duties. The Consultant will: (I) Provide and perform tasks identified in Attachment A, Scope of Services. (2) Provide miscellaneous consulting and other services related to the Project and not stipulated elsewhere in this Task Order, when requested by the City in writing. A Task Order Amendment is required if these additional services cannot be performed within the Task Order budget. 2. Date of completion; Work hours estimate; Cost estimate and bud -get; Fee schedule. May 2013 City of San Rafael Master Consultant Services Agreement Task Order With regard to each project given to Consultant that is subject to this Task Order, the scope of work and cost estimate are set forth in Attachment A to this Task Order. 3. Authorization to proceed. The City will give Consultant authorization to proceed with the work specified in this Task Order only after Consultant submits updated insurance certificates and endorsements, in accordance with Section 10 of the Agreement. 4. Master Agreement terms; Conflicts. Except as expressly otherwise provided herein, all terms of the Agreement shall apply to Consultant in the performance of work pursuant to this Task Order. If there is a conflict between this Task Order and the Agreement, this Task Order shall control. 5. Amendment of Task Order; Conflicts. Except as provided in Section 1(b)(2) above, this Task Order may not be modified except by an Amended Task Order executed and approved in the same manner as the Task Order. 6. Signatures. The parties have executed this Task Order, as of the date first written above. Matt B Title: Secretary Exhibit A k6A MARK THOMAS May 19, 2020 Theo Sanchez Project No.: SA -19179 City of San Rafael Department of Public Works 111 Morphew Street San Rafael, CA 94901 RE: Proposal for Culvert Inspection and Repair Strategy for Lateral under B Street Dear Theo: Thank you for the opportunity to provide you with this letter proposal to evaluate the existing culvert under B Street under our Master Services Agreement. Based on the information you have provided, our understanding is that the existing 5 -foot wide by 4 -foot high reinforced concrete box culvert lateral running under B Street roughly 50 feet south of San Rafael Creek has a 2 -foot diameter hole in the roof slab. The cause of the damage is unknown, but the City suspects it could be a result of large vehicles (garbage trucks) from the nearby business driving over the curb and coming down on the road. A steel plate has been placed over the hole as a temporary measure. We propose the following scope of work and associated fee to complete the damage assessment and provide recommendation for repair. Scope of Work Mark Thomas will coordinate with the City to complete an inspection of the inside of the culvert. It is assumed that during low tide the culvert will be accessible using waders with no more than a few inches of water flowing in it. Based on our inspection and a visual observation of the site conditions, we will make a determination as to the probable cause of the damage and propose a repair strategy. This will be documented in the form of an Assessment Memorandum that will be submitted to the City. As a future optional task, Mark Thomas can prepare design drawings that detail the repair strategy and provide traffic control plans if traffic needs to be shifted or diverted. We can provide a fee for this work after the extent of the repair is known. Fee Mark Thomas proposes to complete this scope of work for a fee of five thousand dollars ($5,000). Thank you again for the opportunity to work with you on this project.
